ADULT TRANSITION PROGRAM

Our Adult Transitional Programs caters to young adults transitioning from school into independent living. They provide resources, recommendations and services to create and monitor individualized daily schedules.

Transitional programs will include:

  • Activities of daily Living Skills (community and home based)

  • Social Skills (virtual and in-person)

  • Leisure Skills (individual and group)

  • Vocational Skills (volunteer and paid)

  • Health and Wellness (individual and group)

 

The goal of Pivot Therapy's Adult Transitional Program is to incorporate clients' strengths and interests into their daily schedules. Autistic individuals are empowered to gain and maintain their independence when engaging in meaningful activities.

Pivotherapy utilizes screening tools and interest inventories to help identify clients' strengths and interests to assist in creating these individualized transition plans.

 

Clients are required to participate in a 3 month trial period to ensure the safety of all of our participants. Aggressive or eloping behaviors are cause for immediate termination of programming services.

Pivot Therapy's Adult Transitional Program includes:

  • Utilizing screening and inventory tools to research activities of daily living, social, leisure and vocational skill opportunities

  • Assisting in setting up balanced weekly schedules

  • Recording progress

  • Managing necessary supports and opportunities

  • Adapting the environment to meet clients' needs

  • Facilitating communication between providers and caregivers​​​

JACK'S PROGRAM:

Jack was diagnosed with Autism when he was 15 months old. Jack is making incredible strides towards independence by empowering  him to make his own life choices and individualizing his transitional program towards his interests.
